  • Dental Coverage Breakdown by Patient
  • Explanation of Benefits and Payments
  • Estimate/Pre-Determination Status

You will be asked to provide the following in order to authenticate and then access specific patient information on the GroupHEALTH Dental Provider Portal:

  • Certificate Number
  • Birthdate of Patient
  • CDA Billing Office Number
